Complete application and submit required documentation (low-income documentation and proof of rabies vaccine). All documentation can be emailed to us at alteredtailsproject@gmail.com

If application is approved, our organization will reach out to receive payment.

Once payment is received, you will be contacted within 5-7 business days by our organization to schedule your animal’s surgery.
